Improved formatting of saved PATCHS requests that also includes result of AI questionnaire's

We save the request as attachment and the text is badly formatted with lots of extraneous HTML type formatting and doesn't include linked questionnaire's generated by AI.

Single tab for Patches, rather than a new one opening with each patient
