Show patients their PDS status

When a patient views Patchs, query their record in PDS. If they are not found at the practice, display a message to the patient letting them know.

Only include 'Active' rota types on appointment booking list

when sending a patient a self-booking link, the options of the rota types show deactivated rotas as well as active. Please can it only show active rotas otherwise, the list is very long and confusing.
