Allow vulnerable patients access to log a clinical request even when the message cap is reached Merged

Provide a list of patients access to log clinical requests via PATCHs outside of the request cap. These patients would be the ones who had trouble using the telephone, e.g. those with problems with hearing or speech. The list of patients could be uploaded via a csv file and updated regularly to keep the list current.
