Sicknote Requests

When submitting a request for a sicknote could it be separate from other admin requests? As it currently stands patients do not submit enough information and we are constantly replying to patient to request further details. Such as: Is it a new sicknote? Is it an extension? Reason for sicknote? How long do you need your sicknote for? Thanks

Please can we add Sick note date and reason requests to main body of questions as this does not save in the NJ and is a real issue when it comes to doing them thanks
