Add MRC score and number of exacerbations to patient record from CAT COPD questionnaire

When patients are filling out the form it would be useful that the code for the MRC score is also uploaded in to the patient record. Other items such as COPD review and the actual cat score are uploaded sonot sure why the MRC and Number of exacerbations cant also be uploaded.
