Duplicate Patient management and report

We have many duplicate patients, so often the Patchs is in the 'wrong' record - ie NOT the one linked to EMIS.

1. Staff allocating need a warning there is a possible duplicate but they don't, so errors happen daily.

2. There should be a report to find all the duplicates - should be easy as you can see them in patient registration report but needs scrolling through and identifying visually - a report would be available for potential duplicates and easy access to merge function from that.

3. It should also not be possible for patients to add additional accounts - they should be warned there is a possible entry already.
