Show the role of the person who is responding to a request as well as their name

Show the role of the staff user who is responding to a request so that patients don't get confused when several staff members respond.

Only include 'Active' rota types on appointment booking list

when sending a patient a self-booking link, the options of the rota types show deactivated rotas as well as active. Please can it only show active rotas otherwise, the list is very long and confusing.
