Update Wording of 'Sex assigned at birth*' question

On the patient registration page, please change the registration form to ask for

'Sex as it appears in your medical record (i.e. male, female or indeterminate or “I” which is reflected as “Not specified” on PDS)

rather than

'Sex assigned as birth'
