change the task system on patchs to make it more streamlined and standardised

what we are proposing is that we could do away with the one big text box and replace it with tick boxes for who the patient needs to be seen by and in what timescale. Then, text boxes which would allow for elaboration if needed for things like physio or pharmacist requests and a text box for further instructions like the need for tests to be organised before appointments or such like. Attached is an example of what the screen would look like
