Fit note questionnaire amendment Merged

Is it possible to please amend the fit note questionnaire, so that the answer for the question "when do you need this fit note from" only allows the patient to enter the answer in date format?

At the moment it's free text, so some patients aren't providing this information which means we need to go back to the patient to ask for this information, which delays us reviewing the fit note request.

Please can you let me know.
