Self-Booking Change

When a patient uses the self booking function, the slot has no additional information in the booked appointment on the clinical system.

Could we have something that is along the lines of 'Please add a short brief description of the issue/reason for booking this appointment'

This will help clinicians and admin staff know why the appointment is happening at first glance, rather than to dig around first.
