Currently you have radio buttons to allow multiple choices but only one option is allowed. MS Teams forms allow a range of answers and question formats including free text, but these are harder to get the information governance right for than a patchs linked account and so I would like more functionality in the questionnaire writing option. I am writing an ADHD questionnaire for patients to complete to simplify their referrals and give better information to the specialists for prioritisation. I would like the option to have multiple answers for example: Did you have difficulties in Primary School/ Secondary School/ Higher Education. Do you have problems with relationships with Family/ friends/ in work etc.
