Remove "Completed by" feature

Our practice use patchs in a way where they get triaged the next working morning by the doctor, however, we save them to patient records the day before. We then get a lot of patients calling up saying that there request has been completed by [insert staff's name]when it has not actually been actioned yet. This is rather confusing for the patient and they are disheartened when we say that their request is not completed. It was be useful to not notify the patient that their request is completed and by whom.
