linking patchs medication request to patient records so patients can see the medication list and request

currently we ask patients to complete a patchs for clinical problems and go onto patient access for medication requests which confuses them. It would be much better to direct them to one site such as Patchs so they can do clinical or medication requests via one app rather that having to log in to more than one.
