Automatically hide/remove deceased and de-registered patients

Patchs will automatically detect patients that have been marked as deceased or de-registered.
Patchs will automatically remove the patient from the practice patient list within Patchs, blocking them from logging in or sending in requests.
The GP Practice will be able to restore the patient account as with what current manual removal allows.
