Links for individual Patchs request types that can be displayed on a GP practice website

As a GP practice

On my GP practice website

I would like to be able to list links to individual Patchs request types

Whereby if a patient clicks a link

They are taken through to Patchs to submit that specific request type.

Currently the request types are listed on the make a request page, and it is not currently possible to link to each type separately.
