Patient Feedback - checkboxes for consent to share

The practice would like to suggest a checkbox on the patient feedback which allows patient to consent to allow feedback to be shared either anonymously, or with their names attached.


The practice uses this with their current feedback provider to provide consent to share comments on their website, or digital scrolling screens in their reception.
