Different salutations / greetings for patients instead of simply 'Hi'

Practices can set their own default greetings for messages e.g. formal 'Dear Mr / Ms' and/or set these for certain patients e.g. younger patients informal 'Hi...' and older patients formal 'Dear...'

    This doesn't answer the question! How do you set it so that messages don't automatically say "Hi" (which is so inappropriate for so many of our patients) and have it default to "Dear [title] patient's full name"? And not part of any message template, but as the default.
