questionnaire where the answer dictates the next question

The ability to create a questionnaire where the answer dictates the next question. eg

Sick Note


Have you been issued a sick previously for this ailment?


-- "New Sick Note" [button]

-- "Extension to previous sick note" [Button]


then if the patient selects 'Extension to previous sick note', the next question would be


-- Please enter the dates stated on the previous sicknote & the ailment


Whereas, if the patient selects 'New Sick Note' , the next would be "Have you seen by a clinician regarding this ailment at you GP practice or Hospital?


-- "Clinician at Surgery (primary care)" [button]

-- "Clinician at Hospital (secondary care)" [button]

-- "Not seen by any Clinician" [button]
