QOF template

It would be useful to have a single questionnaire which covers some of the main reoccurring QOF themes - Blood pressure readings, smoking status, alcohol status, and height/ weight. This would allow patients to submit general health data in one easy questionnaire - reducing the burden on patients to complete forms and on surgeries to send mutliple different templates. it could then be expanded to include asthma/ copd templates at a later date.
