Add "Changes made to existing appointment" as an action taken choice on admin requests.

"Booked a pre-agreed appointment" is pretty close, but not entirely accurate as the patient already had an appointment, you could use "gave patient information", however it might be nicer for audit to show these seperately from people requesting appointment changes.
