My Health ability to have own questionnaires available

Move 'Eye' Icon for another staff member viewing form to same side as flags and text.

'No requests made' entries

Within 'Patients' tab there are many 'No requests made' entries. Does this relate to a patient ceasing the request mid way? Also, is there any reason this information should appear?
